Our Disease Management program can help pregnant or chronically ill employees experience a better quality of life and may reduce overall health care costs.
Disease management "coaches" provide 24/7 telephonic support, referrals for educational programs, physician interaction and decision support and self-care guidance. All St. John's care management nurses are clinically-experienced RN's, and a majority of our disease management programs are accredited by the National Committee for Quality Assurance.
